What comes after the Heimlich maneuver

At the end of an otherwise informative article about the nuances of performing a Heimlich maneuver, New York Times science reporter Jane E. Brody recommends that if all else fails, a cricothyrotomy should be attempted.

She goes on to briefly explain how the procedure is done. In the right hands, a cricothyrotomy is safer and easier to perform than a formal tracheostomy. However, for a layperson who has never seen either procedure done, does not know the relevant anatomy, and has never put a knife to anyones skin, it is highly unlikely to be successful.

Ms. Brody includes a link to website with some static drawings of the procedure. The site is called Aarons Tracheostomy Page and it bills itself as "The Internets leading tracheostomy resource since 1996."

Heres an excerpt from that description of the operation:

"3. Take the razor blade or knife and make a half-inch horizontal incision. The cut should be about half an inch deep. There should not be too much blood." Yes, there should not be too much blood, but sometimes there is.

Both the Times article and the reference repeat the medical urban legend that the barrel of a ballpoint pen can be used as a breathing tube.

A 2010 paper found that due to high resistance to airflow, most ballpoint pens are not adequate airways, and the two that were acceptable (the Baron retractable ballpoint and the BIC Soft Feel Jumbo) are unlikely to be on hand. An earlier paper also reported similar high airflow resistance with ballpoint pens.

A small study involving inexperienced junior doctors and medical students found that they were able to successfully perform cricothyrotomies in only 8 of 14 cadavers. Injuries to the thyroid and cricoid cartilages were common.

Remember these important points—cadavers dont need an airway in a hurry and they dont bleed.

Evidence of successful cricothyrotomy by bystanders is lacking. A 2010 review of American soldiers killed in Iraq between 2003 and 2006 noted that five of those who died appeared to have had attempts at cricothyrotomy, all of which failed.

I once was asked to see a patient whose "cricothyrotomy" done in an ED by an experienced emergency physician and a resident turned out to be a laryngotomy. The tube was inserted directly into the larynx.

To the uninitiated, surgery looks easy. Last year I blogged about Malcolm Gladwells outrageous claim that just about any college graduate could become a cardiac surgeon.

I suppose one might say "What have you got to lose? The patient is dying. Try the cricothyrotomy." I can’t stop you. But be certain it is necessary, and realize your chances of success are extremely low.

If you’re considering it, at least look at some of the many instructional videos available online.

Warning: Graphic. There is some blood. Here’s one by an ED doc. In a non-hospital setting, you would not have all the help and equipment he had. Here’s another, this time by a surgeon—with lots of help and equipment. Both patients were relatively thin.

Now imagine doing it with a pocket knife and a ballpoint pen on an obese person. Still think it’s easy?

Some venous thromboembolic events can’t be prevented even with optimal care

I have written several posts about how I get things right before others see the light, but none better than one from three years ago pointing out that some of the Centers for Medicare and Medicaid Services (CMS) "never events" cant really be completely prevented and therefore should not be considered "never events."

One specific "never event" I questioned was hospital acquired venous thromboembolic (VTE) disease which encompasses deep venous thrombosis (DVT) and/or pulmonary embolism (PE). I wrote "I am unaware of any DVT study in which no patients in the experimental arm developed DVTs or PEs. Patients will develop DVT or PE even with the best evidence-based care."

Along comes a brief research letter published last month in JAMA Surgery by a group from Johns Hopkins led by surgeon Elliott R. Haut.

Of 92 patients in their institution who had VTEs in a single year, 43 (47%) had received defect-free care. That is, each of those patients received all doses of risk-appropriate pharmacological prophylaxis ordered for the entire hospitalization.

To put it another way, VTEs for those 43 patients were not preventable. There would be no way to do a quality improvement project for a group of patients who received the right prophylaxis throughout their hospital stays and still got VTEs.

The Joint Commission/CMS criterion states that a hospital is in compliance with VTE prophylaxis if a patient receives one dose of an appropriate drug within 24 hours of admission. The Hopkins study showed that of the 49 patients (53%) whose care was suboptimal, 36 (73%) missed at least one dose of prophylaxis that was correctly ordered. Other studies have shown that missing even one dose of prophylaxis at any time during a hospitalization increases the risk of VTE.

So about half of VTEs are not preventable even with perfect adherence to the prophylaxis protocol, and the standard for compliance established by the JC/CMS is inadequate to judge the quality of an institutions performance for VTE prevention.

The study shows that 1) a lot of good information can be delivered in a two-page paper, 2) JC/CMS criteria for compliance with VTE prophylaxis need to be revisited, and 3) VTE should be removed from the list of "never events.”

Supergirl Pilot Leaked! A Review Some Spoilers

The highly anticipated live action Supergirl series will premiere November 2015. Today (May 22, 2015) the pilot was leaked online onto numerous Torrent Sites in HD quality. News of the leak spread like wildfire on the internet and many fans gave into their curiosity. The response? Overwhelmingly positive. The anticipation was killing me so I decided to watch it for myself. And I loved it.



Theres a lot of speculation if the leak was in fact piracy or a marketing ploy by CBS (who has not issued a statement regarding the video). Whatever the case, Im grateful for a glimpse into the series, 6 months before it debuts. First and foremost its a wonderful thing that a female superhero is headlining her own show. For decades the superhero market (movies and television) have been centered around men and the female themed ones turned into disasters (Catwoman and Elektra to name a few). Black Widow has essentially been blacklisted by Disney and was wiped from Age of Ultron merchandise. Young girls everywhere are craving for a female hero. I have a first hand look in seeing how excited little girls get at charity events when they spot me as Supergirl or Wonder Woman. I dont want to give too much away but the series starts out with Kara being sent to Earth to protect her cousin Kal-El right before the explosion of Krypton. We witness everything from her first encounter with Superman, her adoption, her job in National City, her relationship with her boss and coworkers to her rescuing a fiery airplane. Thus #Supergirl is born (complete with hashtag) and she faces her first adversary; a criminal from an alien prison.

The show has special effects worthy of the big screen. The script is solid and dives right into the mythology and action from the first frame. This is a great introduction into the series and as a life long fan of Supergirl I was very satisfied. The best thing about this show is that its family friendly without being corny. Supergirls integrity has remained intact. I was worried that in an effort to make her cool and trendy she would be given a darker, brooding makeover like so many other superheros throughout the years. Even Man of Steel was a darker incarnation of Superman and judging by the Dawn of Justice trailer, the next film will be even darker. This is not the case with Supergirl. Its exciting and brings hope, not depression. She is someone you are rooting for. I am unfamiliar with Melissa Benoist (who plays Kara Zor-El/Supergirl) previous work and was very curious on how she would do as this iconic character. I can say she was adorable and amazing. A perfect combination of the everyday girl learning to accept her inner hero. She gets two very big thumbs up from me as does the pilot.
